Our membership plans are designed to help individuals and families maintain consistent preventive care while also enjoying savings on additional treatment throughout the year.
Unlike traditional dental insurance, our plans come with no deductibles, waiting periods, annual maximums, or complicated claim forms. Everything is kept simple and straightforward, so you can focus on your oral health with predictable, affordable care. Our in-house membership options are built for patients of all ages, supporting long-term dental health and overall wellness.
Adult Healthy Prevention Plan
($365 Annually)
This plan is designed for our patients with generally healthy gums who want to maintain routine preventive care and overall oral health.
Includes:
- 2 professional dental cleanings per year
- 2 doctor exams per year
- 1 full-mouth X-ray series per year
- 1 emergency exam per year (if needed)
- Fluoride treatment
- Intraoral photographs and orthodontic records
- Oral cancer screening
- 20% off most additional treatments
- 12% off Invisalign (lab fee of $2,000 applies)
- 20% off when using CareCredit, Sunbit, or outside financing
Adult Periodontal Maintenance Plan
($450 Annually)
Designed for patients diagnosed with periodontal disease who require more frequent maintenance care.
Includes:
- 3 periodontal cleanings per year
- 2 doctor exams per year
- 1 full-mouth X-ray series per year (additional as needed)
- 1 emergency exam per year (if needed)
- Fluoride treatment
- Intraoral photographs and orthodontic records
- Oral cancer screening
- 20% off most additional treatments
- 20% off localized periodontal antibiotic therapy
- 12% off Invisalign (lab fee of $2,000 applies)
- 20% off when using CareCredit, Sunbit, or outside financing
Membership Terms & Conditions
- No annual deductibles
- No annual maximums
- No preauthorization requirements
- No waiting or eligibility periods for treatment
- No missing tooth clause restrictions or exclusions
- Annual enrollment fee is non-refundable
- Payment in full is required at each visit to maintain benefits
- Membership is non-transferable and cannot be shared or reassigned
- Cannot be combined with insurance, discount plans, or other offers
- Fees are based on practice UCR rates and may be subject to change
- Membership must remain active to receive benefits
- Patients are responsible for scheduling and maintaining appointments
Appointment Policy
Our team reserves time specifically for each patient to ensure quality care. We require at least 48 hours’ notice (two business days) for cancellations or rescheduling.
Failure to provide proper notice may result in:
- Loss of a scheduled hygiene visit, or
- A missed appointment fee of at least $50 per hour reserved
Repeated missed appointments may result in written warnings. After two warnings, membership and/or treatment privileges may be terminated without refund.
Emergencies and illnesses are exceptions, but patients must contact the office as soon as possible to reschedule.
Additional Program Guidelines & Limitations
This membership plan is only valid at Broadwing Dental Studio and is not insurance. It is a wellness-based payment program that allows patients to prepay annually for preventive care and receive discounted treatment fees.
Important limitations include:
- Not valid for specialist care (orthodontics, periodontics, endodontics, oral surgery, pediatric dentistry referrals)
- Cosmetic and complex restorative treatments may require additional diagnostic or lab fees
- Cannot be used with any other insurance, discount plan, or promotional offer
- Not valid at other dental offices
- Discounts cannot be combined with other offers or programs
- Additional radiographs are determined by your dentist at the time of service
- Fees are non-refundable for non-use, relocation, or obtaining insurance
- Plan automatically renews annually unless canceled
- Treatment fees are guaranteed for 30 days from the date quoted
- Membership may be terminated for delinquency or inappropriate conduct
- Termination may occur after two no-show appointments
- All payments are due at time of service to receive discounted pricing
- Invisalign services receive a 12% discount
- Financing via CareCredit, Sunbit, or outside lenders qualifies for a 20% discount
